PREPARE FOR YOUR VISIT

Most of the people we work with have never participated in a neuropsychological evaluation. We want you to be comfortable with our process, and we find that anxiety is reduced when patient's know a little more about what we will be doing during our time together. In addition to the information here about what to expect and what to bring, at our first appointment, we will talk more about how we will be working with you.

What to Expect at Your Appointment

When scheduling your initial appointment with our office, you will be sent either paper copies of our intake paperwork or provided with a secure link to complete them electronically. The paperwork needs to be completed at least 72 hours prior to your initial appointment with our office. If your forms are completed electronically, we will be notified that they are complete and ready for review. If you are sent paper copies, the forms need to be mailed or faxed back to our office.


The appointment may need to be rescheduled if we do not receive the completed paper work prior to your appointment.


Please check in 10 to 15 minutes prior to your appointments.


Refrain from smoking cigarettes or marijuana prior to your appointment. We also ask that patients and any individual accompanying the patient to our office refrain from wearing cologne, perfume, or scented lotions to the office.


With the exception of documented service animals, animals of any kind are not allowed in the office.



What to Bring

  • All insurance cards.
  • Driver’s license or other form of identification.
  • A list of your current medications. Please only bring a list, as we will not go through your medication bottles.
  • Reading glasses, hearing aids, or other assistive devices.

Our Process

Most evaluations are broken into two to three appointments. We have an initial meeting during which your provider will review your concerns, background, medical history, etc. We then see you for an evaluation appointment. The evaluation appointment typically lasts 2 1/2 to 3 hours, though this varies depending on the reason for referral and individual patient needs. A third appointment is typically offered to review the findings and treatment recommendations.

Neuropsychological Services of Oregon, LLC (NSO) provides neuropsychological and psychological services to adults. The goal of the clinical staff at NSO is to complete a comprehensive evaluation that may assist with diagnostic clarification and treatment planning.
